Taylor Swift ‘Patient Zero’ Video Behind-the-Scenes

Taylor Swift released an eight-minute expanded video showing how she directed “Patient Zero,” with behind-the-scenes footage of her working alongside Dakota Johnson and Colin Farrell.

Swift posted a link to the Patient Zero (Expanded Video) on her Instagram Stories on Saturday (Oct. 3), pointing fans to Apple Music, where the clip is available as part of a video bundle that includes the song and the original music video.

What the expanded video reveals

In the eight-minute behind-the-scenes footage, Swift breaks down her creative vision from the start. “I’ve been wanting to make this video my entire life,” she says, describing her love of “sleek” films set in a “monochromatic” world where the stakes remain mysterious until the very end.

Swift explains her influences: “You don’t quite understand the world, and you don’t understand what’s wrong with this couple until you do. Meets ghost story, meets true crime, meets a lot of the plot lines I’ve seen on Dateline.”

The expanded video shows Swift comfortable and energized behind the camera, directing Johnson and Farrell through the video’s eerie scenes. She gives a shout-out to cinematographer Emmanuel “El Chivo” Lubezki, saying: “His name is Chivo. They call him that because it means goat, ’cause he’s the f, king goat.”

The cast weighs in

Johnson discusses the experience of collaborating with Swift, her friend off-camera. “Sometimes when you work with your friends it can really go south, and it will affect your friendship,” she says, according to Billboard. “But she and I work really well together.”

Farrell, who portrays a narcissistic and ambitious character in the video, praises Swift’s direction, per Pitchfork. “The joy she has with her creative engine, which is a really significant creative engine, the joy she has with it is really quite humbling, actually. It’s very moving,” he says.

The song and video concept

The “Patient Zero” music video premiered at the MTV Video Music Awards on Sept. 27, directed entirely by Swift. The clip follows Johnson as a woman whose health deteriorates during her marriage to Farrell’s character, while Swift appears as the ghost of his former wife, warning Johnson about his past and revealing a dark twist.

“Patient Zero” is one of four new songs Swift released Friday as part of The Life of a Showgirl: The Encore, an expanded edition of her 2025 album. The other bonus tracks are “Cleveland!,” “Pink Clouding,” and “Babylon.”

Swift also won the inaugural Artist-Director Honor at the 2026 VMAs, recognizing her work behind the camera for expanding the possibilities of music video and advancing visual storytelling. She won Video of the Year for “The Fate of Ophelia” at the same ceremony. During her acceptance speech, she thanked fans for making music videos rewarding and noted she has been part of making roughly 60 music videos over 20 years, many of which she wrote and directed.
